I managed to sprain my ankle when I was in grade 6 or 7 (not sure) during a game of dodgeball. I actually can't remember it happening, so I asked one of my classmates who was fussing over me and she said there was a crack noise and then I was on the gym floor laughing my ass off. I was fine after a day or two.
I think it was in December 2009, Christmas Eve, when I got my right pinkie slammed on by/stuck in a loading dock at the grocery store I worked at. Very very painful, lots of bleeding. Oddly enough, the bone didn't break, but the nail had actually been torn off (though I didn't know until after the New Year when it easily and painlessly came off). Now, my pinkie is normal, except that I lost a little bit of feeling in it.
Last Thursday, July 26th, I hurt my back on the job. It was really painful and bad enough that I needed to push down on the belt loops of my shorts to stand up straight. Luckily there was a walk-in medical clinic just on the other side of the strip-mall. I was really worried that I had hurt my spine, but after the doctor did a few tests (it was entertaining and painful because I am ticklish) he found out that I had just pulled a muscle in my back. He prescribed some painkillers, gave me a note to stay off the job until Wednesday, and he wants to see me again this Tuesday. It's getting better.
